PARTICIPATING RESTAURANT AGREEMENT

AS A PARTICIPANT, THIS RESTAURANT AGREES TO:
  1. Contribute a minimum of 25% of each patron’s food and non-alcoholic beverage bill on 5/1/2008 (including all meals served that day) to Project Angel Heart.
  2. Allow Project Angel Heart to use this restaurant’s name in materials promoting Dining Out for Life®.
  3. Promote Dining Out for Life® by displaying and/or distributing event materials at least 2 weeks before the event.
  4. Inform wait staff, managers, and other restaurant personnel about Dining Out for Life® so that they can promote the event to diners.
  5. Permit Project Angel Heart to place a volunteer ambassador in your restaurant on Dining Out for Life® night. Ambassadors will approach tables to thank guests, share information, answer questions and solicit donations/shopping spree drawing entry envelopes.
  6. Send or deliver a check for the total contribution payable to Project Angel Heart by no later than June 1, 2008.

PROJECT ANGEL HEART WILL:
  1. Produce, utilize and distribute attractive, high quality promotional materials including posters, bill stuffers and postcards, totaling over 340,000 pieces.
  2. Orchestrate a publicity campaign to promote Dining Out for Life® via local radio, TV and print media.
  3. Mail Dining Out for Life® postcard or flyer with complete list of participating restaurant names and telephone numbers to over 30,000 supporters.
  4. Establish phone and website referral list of participating restaurants for diners.
  5. Provide periodic communication with your restaurant about Dining Out for Life®.

Volunteer ambassadors are vitally important in promoting your restaurant’s participation on the day of the event. Ambassadors will approach tables to thank guests, share information, answer questions and solicit donation/shopping spree drawing entry envelopes. In some cases we do not have enough ambassadors for every restaurant. Additional Promotional Opportunities:
Throughout the Dining Out for Life® planning program, there are several opportunities to promote your restaurant and its participation in Dining Out for Life®. If you wish to participate in any of the following opportunities or would like information, please check the appropriate boxes. A Project Angel Heart staff member will be contacting you regarding your interest.*
Returning ambassador training site: Host a returning ambassador training providing appetizers and non-alcoholic beverages. Trainings last between 1-2 hours. Approximately 30 ambassadors.
Radio promotional events: Provide restaurant gift certificates to be distributed through media promotions. Must be at least five separate gift certificates.
Food service promotional events: Be available to supply food to media locations through on-site promotions or live remotes.
Ambassador recruitment and training: Supply gift certificates to assist in volunteer recruitment and incentive program. A minimum of one certificate.


* Project Angel Heart will provide a separate receipt for any in-kind donations during Dining Out for Life®.
